Nurse Care Consultant/Clinical Care:

Our Care Consultant position is open to RN applicants, and New Grads welcome to apply! The Care Consultant will perform assessments, build care plans, supervise care delivery and manage our clients' overall care experience. Another skill necessary is the ability to mentor nurses and collaborate with caregivers. This role is Full-Time, salaried, mix of clinical and administrative responsibilities, with a regular work week of Monday thru Friday. Your day will be working in office building with administrative team, while also working in the field for Clinical duties such as, patient assessments and case management island-wide on Oahu. This role involves mentoring nurses and collaborating with caregivers

Responsibilities

  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Collaborate with healthcare teams to improve patient care quality and safety.
  • Provide expert advice on healthcare policies and clinical procedures.
  • Develop and implement staff training and education programs.
  • Monitor patient outcomes and suggest improvements.
  • Assist in the development of healthcare policies and guidelines.
  • Serve as a liaison between administrative and clinical teams.
  • Facilitate communication among multidisciplinary teams.

Requirements for employment:

- State of Hawaii Registered Nurse license, current and in good standing
- Current CPR & First Aid Certifications
- Current TB clearance + proof of past TB 2-step test
- Ability to ambulate within a variety of home environments
- Criminal Abstract will be reviewed
- Pass written entry exams and skills assessments
- Valid driver's license

License/Certification:
